FOUNDATIONERS' "KEEP IN TOUCH" APPLICATION FORM

If you would like to keep in touch we would like to send you an occasional newsletter or other news about the Foundation. To do this we need to keep some of your details on file - name, school attended (and dates), your date of birth, family or current address (or the address we have on file), home phone, mobile numbers and an e-mail address if you have one. The Data Protection Act requires us to seek permission to keep your details on record. If you would like to keep in touch, please indicate your agreement by selecting the "yes" tickbox.
